I remember a recent thread here somewhere where people were talking about questions asked on visitation forms. I couldnt find that thread so I thought I would start a new one. Anyways, my question is what type of questions do they ask on the forms? I know it would vary by state, but looking for info about Michigan. Think it would be nice to hear about other states to.
