Smiley, you can most likely find plenty of information on the internet (or even a local hospital) to you and get the forms and send those to your pal. If he's in the states, I think he will be able to do it on his own. It will ask all those questions about if he's ever on life support and also if he wants a DNR (do not resuscitate order). Some of it might be a little complicated but he should be able to ask the education building/library in the prison there if he needs clarification on any of the questions. Its all accessible.
